What does the service provide for end of life care patients and their carers?
St. Margaret’s Hospice at Home Team is a team of experienced Health Care
Assistants who provide a free specialist home nursing service to adults over
the age of 18 in the last stages of a terminal illness living in Somerset. In
addition to providing nursing care, the Health Care Assistants are also
available to provide emotional support and practical advice to carers and
families.
The service complements and supports existing St. Margaret’s Hospice
services and is integral to a multi-professional and multi-agency approach facilitating
the provision of 24-hour care based on individual need. Care is provided with
support and supervision from the Community Palliative Care Nurse Specialists
and the Advice Line at St. Margaret’s Hospice, as well as from District
Nursing colleagues. The Hospice at Home Health Care Assistants will wear
uniform and carry appropriate identity. Any changes observed in the patient’s
condition will be reported to a Registered Nurse or Doctor.
How do patients and carers access this service?
Patients can access this service via the district nursing team or a hospital/community palliative care nurse specialist.
Hours of availability
St Margaret’s Hospice at Home can provide care at anytime of the
day or night.
Shifts can
be booked via St Margaret’s central referral centre or the end of life
care coordination centre but can also be booked by the district nursing team
and hospital/community palliative care teams during the period Monday – Friday
between 09:00 – 17:00.
